[*] Scope of Service:

(1) Vendor needs to provide document scanning and shredding service.
- Ability to index box contents on behalf of department prior to processing.
- Box to be labelled by originating location and number.
- Each multi-page document to be extracted from box.
- Existing staples and paper clips to be removed.
- Pages to be scanned using optimal scanner settings based on condition.
- Resulting PDF file to be named based on the batch or folder description (tab, label, etc.).
- If batch or folder descriptor is non-existent, vendor will use a filename best representing the document content.
- Each document to be placed back in box preserving initial order.
- Vendor chain of custody to be documented shredding requirements:
- Ability to index box contents on behalf of department prior to processing.
- Vendor has proof of HIPAA compliance vendor has proof of IRS publication 1075 compliance.
- Destroy paper using crosscut shredders which produce particles that are 1 mm x 5 mm (0.04 in. x 0.2 in.) in size (or smaller) or pulverize or disintegrate paper materials using disintegrator devices equipped with a 3/32 in. (2.4 mm) security screen.
- Transportation and processing areas are secured to authorized employees. Employees are bonded and have background checks.
- Vendor must destroy records within 24 hours of pick-up and chain of custody is documented.
(2) An optional pre-proposal conference will be held on June 02, 2023.
(3) All question must be submitted no later than June 23, 2023.
