[*] Scope of Service:

(1) Vendor needs to provide reproductive health care and related preventive health services to the government authority located in Honolulu, HI.
- demographics including an estimated number of clients in need of reproductive health services and number of uninsured or under-insured clients to be served, unique characteristics of the specific populations that impact access to services and geographic area to be served; and,
- Evidence that the proposed program addresses the reproductive health care and related preventive health services in these areas including the provision of a broad range of acceptable and effective family planning methods and services, and discussion of any existing resources
- The applicant shall provide individuals upon request, a range of factual, medically accurate information to support informed decisions on reproductive health care and information of available resources,
- The applicant shall document services provided in the client’s record,
- The applicant shall have in place or be in the process of developing and able to assure the implementation of an electronic health record keeping system for all clients,
- The applicant shall have protocols in place and staff training ensuring medical records are encrypted, password protected, confidential and safeguarded against loss or unauthorized use or access.
(2) All the questions must be submitted no later than April 13, 2023.
