- To address health equity gaps experienced by underserved populations through a strategy to utilize existing infrastructure and facilities, maximize opportunities for community access and advance health outcomes
- Annual Wellness Visits
- Advanced Care Planning
- Depression Screenings
- Medicare Preventive Services as defined by Centers for Medicare and Medicaid (CMS)
- Fall Risk Assessments
- Neuro-Cognitive Assessments
- Education and information that equips clients with the knowledge and skills to make healthy behavior changes
(2) A pre-proposal Conference will be held on April 10, 2023.
(3) All question must be submitted no later than April 17, 2023.
