[*] Scope of Service:

(1) Vendor needs to provide third party administrator for health benefit plans service in three areas: group health benefits, flexible spending medical or dependent care and COBRA or HIPPA to the government authority located in Arlington Heights, IL.
- Reimbursement plan with debit card including annual and on-going enrolments, terminations, account balances, weekly claims processing, semi-annual statements, annual enrollment confirmation kits, new hire welcome packages, debit card management and participant inquiries.
- Account balance reconciliation report with pace on bi-weekly basis.
- Provide monthly contribution and payment reports for pace account reconciliation.
- Cobra administration including Cobra notice, eligibility maintenance, enrollment, premium billing, provider notification of enrolments and terminations.
- Collect Cobra premiums.
- Handle all Cobra inquiries.
- Contractor must be HIPAA compliant.
(2) All question must be submitted no later than March 30, 2023.
(3) A contract period will be for five years.
