(1) Vendor needs to provide accountable care organization or direct primary care partner program for its self‐funded employee health plan to the government authority located in Longmont, CO.
- Evaluate and pursue innovative plan designs that contain costs, improve care delivery and use of high value, lower cost services;
- Enhance patient affiliation with primary care providers;
- Improve patient experience and assist employees in navigating the increasingly complex health care system;
- Continue the shift from reactive, episodic care to a continuous care model;
- Maintain a competitive benefit package for employees;
- Continue to improve reporting to provide comprehensive data to monitor program costs and utilization.
(2) All the questions must be submitted no later than February 16, 2023
(3) The contract period will be for one year.
(4) Notice of Intent Deadline must be submitted no later than February 13, 2023
