[*] Scope of Service:

(1) Vendor needs to provide employee health services to the government authority located in Virginia.
- Provide all labor, material, equipment and supervision to provide pre-employment physical examinations, pre-employment urine drug screening and alcohol breath testing, annual physical examinations, random and post*accident urine drug screening, random and post* accident breath alcohol testing, and manual differential lab testing services on an if-and-when required basis during normal business hours and hours outside of normal business hours
- Provide a minimum of one (l) Clinic Director, one (l) full-time Nurse Practitioner or
Physician Assistant, and one (l) Medical Doctor specializing in occupational medicine;
- Provide the necessary equipment at clinic location(s) for performing all required services; 4. Provide test and examination results to the Director, or their designee, from the requesting department (Human Resources or Transportation) within three (3) business days of the day the test or examination was performed;
- Provide tests results to the Director, or their designee, from the requesting department (Human Resources or Transportation), for random and post-accident urine drug screenings and breath alcohol testing on the same day as the test or as soon as the results are available;
- Provide intet-pretation services for the results of testing and physical examinations;
- Maintain and secure all health records of employees serviced;
(2) All questions must be submitted no later than February 15, 2023.
