[*] Scope of Service:

(1) Vendor needs to provide hazard mitigation plan and plan update service to the government authority located in Nashville TN.
- The grantee shall provide applicants and members assistance with creating and maintaining an online account using connect during advocacy, outreach or other forms of services provided under the applicable grant.
- The includes providing applicants and members with assistance selecting the individual’s communication preferences whether by mail, text, email.
- Community outreach shall consist of assisting individuals with enrollment, renewal and establishing Connect accounts as appropriate.
- The grantee shall assist members who receive a renewal packet by helping them to complete the packet and submit requested verifications through any available modality.
(2) All questions must be submitted no later than February 1, 2023.
(3) Contract term will be two years.
