[*] Scope of Service:

(1) Vendor needs to provide nursing home administrator services to the government authority located in Hagerstown, MD.
- Must possess a current state license to practice as a nursing home administrator or willing to work towards immediately
- Tuberculosis:
- Provide proof of administration of a two-step tuberculin skin test.  The tuberculin skin test must be completed within the last 12 months and must contain results recorded in millimetres of induration, date provided of each reading, signature of administrator, and the lot number and manufacturer of the purified protein derivative (PPD) used to be accepted
- Measles, Mumps, Rubella, Varicella and Tdap:
- Immunity is verified by one of the following:
- Documented evidence of administration of the vaccine.
- Laboratory evidence of immunity
- Influenza:
- Annual immunization for influenza, unless refusal due to:
- Medically contraindicated
- Religious beliefs
- The Contractor shall document time actually spent at the Center by signing in on arrival and signing out on departure.
