[B] Scope of Service:

(1) Vendor needs to provide physician services to the government authority located in Kentucky.
- Provide medical consultation, evaluation, medication follow-up and on-call consultations.
- The physician will diagnose and treat illnesses of the student body
- He or She will be available for recommendations for medication, laboratory and X-ray work;  
- admission to hospital facilities and referral to specialist when indicated.
- provide medical, rehabilitative and therapeutic care for students housed and served by the facility.
- Medical supervision is also provided by said physician with regards to injections, medication, wound care, mental health and therapeutic disciplines.
- The physician, assisted by a staff nurse, will complete physical environmental examinations on all incoming consumers with regards to their health and program and training requirements.
- All medical records are kept on site in the medical records library for a period of at least 7 years.
(2) The contract period will be for two years.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
