[B] Scope of Service:

(1) Vendor needs to provide comprehensive medical services for contains medical history, key administrative clinical data relevant to the inmate care received, health care services administered, medication administration, vision screenings, dental care, mental health services, diagnostic and laboratory test results, referral documentation, and clinical treatment records,
- Dental intake screening, intake physical examination, periodic physical examination, intake dental examination, mental health evaluation, health care referral reviews, etc. to the government authority located in Frankfort, KY.
(2) A virtual site visit will be held on February 18, 2021.
(3) All questions must be submitted no later than February 12, 2021.
(4) A contract term will be for three years.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
